1. Indications
Lung transplantation is indicated for patients with chronic, end-stage lung disease who are failing maximal medical therapy, or for whom no effective medical therapy exists. Potential candidates should be well informed and demonstrate adequate health behavior and a willingness to adhere to guidelines from health care professionals.

There are no prospective, randomized, well-powered studies in lung transplantation to support the recommendations in this report. Therefore, the guidelines for referral and transplantation proposed hereafter are, for the most part, a combination of recommendations based on small and/or retrospective and/or registry studies, and expert opinion consensus. Summary
Since the writing of the 1998 guidelines for the selection of candidates for lung transplantation, there has been an increased understanding of the natural history of various lung diseases as well as new treatment strategies developed that may forestall the need for transplantation for certain disorders. This has resulted in several changes to the current strategy for selecting patients for this procedure.
