He had travelled to the UK after residing in multiple European cities. He presented with a 1-week history of right-sided pleuritic chest pain. There was no evidence of a productive cough or recent viral illness and he had no history of injury to the chest. In addition to his chest pain, he had been suffering from persistent right upper quadrant discomfort for approximately 3 years. There was no nausea or vomiting and he denied any bowel or urinary symptoms. He had experienced episodes of fever and loss of appetite but no change in weight. His past medical background consisted of black water fever, as a consequence of malaria, and a previous gunshot wound, which had now healed.
