2021, Respiratory MedicineCitation Excerpt :Using this cutoff, we divided severe asthma patients in two groups, low IgE and high IgE, and majority of severe asthmatics (67%) belongs to high IgE group. We have used cut off level of Eos around 300 cells/μL to determine high Eos [5,14,15], as eosinophils count in induced sputum are rarely available for clinical practice [16]. Katz et al. found that blood eosinophils were a better predictor of treatment response to anti IL-5 treatment compared to sputum eosinophils, speculating that this may be associated with IL-5 mediated eosinophilic inflammation being the result of both local and systemic inflammation [17].

Thus, it is not available in all health centers, as such that other methods have been explored for identifying the eosinophilic phenotype, such as the determination of the fraction of exhaled nitric oxide, periostin levels in serum, or the BEC [15]. Given that the BEC determination is widely available, and is cheaper than the sputum analysis, it could be used as a more effective indicator in the management of asthma.
